Application Forms and Consent Forms
Below is a bunch of application and consent forms that teams have used in the past, they are in Microsoft Word format. Feel free to download and adapt as necessary.

APPLICATION FORMS
	Staff Application Form 1 (Application form to be filled out by members of your team)
	Staff Application Form 2 (Application form to be filled out by members of your team)
	Letter of Interest (A letter you can use seeking expressions of interest for people joining a StormCo team)
	Covering Letter (Covering Letter for Application package which includes application forms, recomendation forms, sponsorship forms)
	Reference Form (This form can be used if you require applicants to seek references)
	Recommendation Form (This form can be used if you require applicants to seek references or recomendation)
	Sponsorship Form (This form can be used by team members seeking sponsorship for a StormCo project)

CONSENT FORMS
	Permission and Medical Record Form (This form contains permission forms for team members who are under 18 years of age and medical record forms that each member should fill out)
	Parent Letter and Consent Form 1 (This letter is written for team members who are from a school)
	Parent Letter 2 (This letter is a follow up letter to the one above)
	Permission Form (This form is for when parent's permission is required because team members are under 18 years of age)
